Release of Liability and Assumption of Risk

Effective Date: June 2026

Important: This is a legally binding document. Please read it carefully before agreeing.

In consideration of being permitted to participate in the Saginaw Area Futsal League ("SAFL") activities, including but not limited to games, practices, tournaments, clinics, camps, and any related events (collectively, "Activities"), I, the undersigned parent or legal guardian of the registered participant ("Participant"), hereby acknowledge, agree, and represent the following:

1. Assumption of Risk

I understand and acknowledge that participation in futsal and related Activities involves inherent risks, dangers, and hazards. These risks include, but are not limited to:

  • Physical injury, including sprains, strains, fractures, concussions, and other serious bodily harm
  • Contact and collision with other players, equipment, walls, flooring, and goal structures
  • Cardiovascular stress and related health events
  • Injuries resulting from slips, trips, and falls on playing surfaces
  • Weather-related risks when Activities take place outdoors
  • Exposure to communicable diseases, including but not limited to COVID-19
  • Travel to and from Activity locations

I voluntarily assume all such risks, both known and unknown, and accept full responsibility for any injury, illness, damage, or loss that may result from participation.

2. Medical Fitness and Insurance

I represent that the Participant is physically fit and has no medical condition, injury, or impairment that would prevent safe participation in the Activities. I acknowledge that it is my responsibility to:

  • Ensure the Participant has appropriate medical insurance coverage
  • Consult with a physician regarding the Participant's fitness for athletic activity
  • Inform SAFL of any relevant medical conditions, allergies, or special needs

3. Release and Waiver of Liability

I hereby release, waive, discharge, and covenant not to sue the Saginaw Area Futsal League, its directors, officers, employees, coaches, volunteers, agents, sponsors, partners, and facility owners (collectively, "Releasees") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury — including death — that may occur as a result of the Participant's involvement in the Activities, whether caused by the negligence of the Releasees or otherwise.

4. Indemnification

I agree to indemnify, defend, and hold harmless the Releasees from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities — including attorney's fees — brought by any third party as a result of the Participant's involvement in the Activities, whether caused by the negligence of the Releasees or otherwise.

5. Emergency Medical Treatment

In the event of an emergency, I hereby grant permission to SAFL representatives and authorized medical personnel to administer or arrange for emergency medical treatment for the Participant. I agree to assume all costs associated with such treatment.

6. Equipment and Facility Use

I acknowledge that the Participant is responsible for using appropriate protective equipment (including shin guards) during all Activities. I understand that SAFL is not responsible for lost, stolen, or damaged personal property brought to Activity locations.

7. Code of Conduct

I agree that the Participant and I will abide by SAFL's Code of Conduct, including respectful behavior toward players, coaches, officials, and spectators. I understand that violations may result in disciplinary action up to and including removal from the league without refund.

8. Severability and Governing Law

If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect. This waiver shall be governed by and construed in accordance with the laws of the State of Michigan.

9. Electronic Signature

By checking the "I agree to the Liability & Participation Waiver" box during the SAFL registration process, I acknowledge that I have read, understand, and voluntarily agree to all terms and conditions set forth in this waiver. I understand that this electronic agreement constitutes a binding legal document and is equivalent to a handwritten signature.

Addendum Notice: This section supplements and modifies the Release of Liability above. If any provision here conflicts with the original waiver, this addendum governs for the Summer 2026 outdoor season.

Summer Outdoor Venue Addendum

Saginaw Area Futsal League (SAFL) · Effective Date: June 2026

1. Scope of This Addendum

This addendum applies specifically to Activities held during the Summer 2026 season at outdoor public park locations used by SAFL (the "Outdoor Venue"), as opposed to the indoor courts and facilities described in the original waiver. By agreeing to this addendum, I acknowledge that the Outdoor Venue presents risks and conditions that differ materially from indoor futsal facilities.

2. Additional Assumption of Risk — Outdoor Public Park

In addition to the risks identified in the original waiver, I understand and acknowledge that the Outdoor Venue involves additional risks, dangers, and hazards specific to a public park setting, including but not limited to:

  • Contact or collision with park infrastructure not designed for athletic play, including metal perimeter fencing, posts, benches, light poles, and other fixed structures that may lack padding or impact protection
  • Uneven, hard, or variable playing surfaces (asphalt, concrete, or ground surfaces) not designed or maintained as athletic courts
  • Exposure to sun, heat, cold, wind, rain, or other weather conditions, including heat-related illness
  • Presence of members of the public, including non-participants, pedestrians, cyclists, and pets, who may access or pass through the Outdoor Venue during Activities
  • Insects, wildlife, uneven terrain, debris, or other natural or environmental hazards typical of an outdoor public space
  • Reduced supervision or control over the boundaries of the playing area compared to an enclosed indoor facility
  • Lack of immediate access to indoor shelter, climate control, or on-site medical facilities

I voluntarily assume all such additional risks, both known and unknown, on the same basis set forth in Section 1 of the original waiver.

3. Acknowledgment of Specific Venue Conditions

I acknowledge that I have been informed the Outdoor Venue includes metal fencing and other park-owned fixtures within or adjacent to the playing area, and that SAFL does not own, control, or maintain the Outdoor Venue. I understand that the governmental or private entity that owns or operates the Outdoor Venue is not a party to the original waiver, and that any claims I may have against that entity are not addressed or released by this addendum or the original waiver.

4. Continued Application of Original Waiver

Except as expressly supplemented by this addendum, all terms of the original SAFL Release of Liability and Assumption of Risk — including the Release and Waiver of Liability, Indemnification, Emergency Medical Treatment, Equipment and Facility Use, Code of Conduct, and Severability and Governing Law provisions — remain in full force and effect and apply equally to Activities held at the Outdoor Venue.

5. Electronic Signature

By checking the "I agree to the Liability & Participation Waiver" box during registration, I acknowledge that I have read, understand, and voluntarily agree to all terms and conditions set forth in this addendum, in addition to the original waiver. I understand that this electronic agreement constitutes a binding legal document and is equivalent to a handwritten signature.
